All the information we discuss is confidential and can only be disclosed to third parties under specific conditions.

Release of Information: This is an optional form you can sign in order for me to communicate (verbally or written) with any third party of your choice. This could include one or several people such as family member(s), a spouse, a friend, or even insurance company. By law, I am not allowed to disclose any information to these parties without your written consent from this form.

Exceptions to confidentiality: According to the law, while everything we discuss is confidential and private, there are certain situations in which I am mandated to break confidentiality. In all of these circumstances, I hope to have conversations with you so that you know what to expect and discuss the best way to keep people protected and safe.

Suspected abuse: If we talk about a child (a person under the age of 18), or elder adult (a person who is 65 or older) who are being neglected or abused, I must involve third parties such as Child Protective Services, Adult Protective Services, or the police to ensure that the people we’ve talked about are helped.

Harm to self or other: If you disclose to me that you’re thinking of hurting yourself or someone else in a life-threatening manner, I will make every effort to work with you to establish safety. However, if we cannot establish this, I must take actions within the means of the law to make sure you or other person(s) are kept safe.
